Companion Services

In the Developmental Disabilities Waiver Services Coverage and Limitations Handbook:
Companion Services consist of non-medical care, supervision and socialization activities provided to an adult on a one-on-one basis or in groups not to exceed three recipients. This service must be provided in direct relation to the achievement of the recipient’s goals on his or her support plan.

In Plain English:
If one of an Individual’s goals is to get out and do things – we assist them to go to museums, to movies, to state parks, to go fishing, to go swimming, whatever they decide that they want to do. The Individual will need to pay for their own meals, tickets, etc., but we provide the transportation and the physical assistance that they need to get out and do stuff. If they are not sure about things that they want to do, we can assist them to find and attend fun stuff for free or very low cost.

In-Home Support Services

In the Developmental Disabilities Waiver Services Coverage and Limitations Handbook:
In-home supports are services that provide recipients who live in their own homes with up to 24 hours-a-day assistance from a support worker or support workers. The support worker provides companionship and personal care and may assist with or perform activities of daily living and other duties necessary to maintain the recipient in supported living. The support worker may perform grocery shopping, housekeeping, and cooking responsibilities or may conduct training programs designed by the supported living coach to teach these and other daily living skills.

In Plain English:
If an Individual wants to live in their own home, the state will pay someone from our company to come to their place and help and assist them in their home and in their town.

Personal Care Assistance

In the Developmental Disabilities Waiver Services Coverage and Limitations Handbook:
Personal care assistance is a service that assists a recipient with eating and meal preparation, bathing, dressing, personal hygiene, and other self-care activities of daily living. The service also includes activities such as assistance with meal preparation, bed making, and vacuuming when these activities are essential to the health, safety, and welfare of the recipient and when no one else is available to perform them.

In Plain English:
If an Individual wants to live in their own home, the state will pay someone from our company to come to their place and help and assist them.

Residential Habilitation Services

In the Developmental Disabilities Waiver Services Coverage and Limitations Handbook:
Residential habilitation provides supervision and specific training activities that assist the recipient to acquire, maintain or improve skills related to activities of daily living. The service focuses on personal hygiene skills such as bathing and oral hygiene; homemaking skills such as food preparation, vacuuming and laundry; and on social and adaptive skills that enable the recipient to reside in the community. This training is provided in accordance with a formal implementation plan, developed with direction from the recipient and reflects the recipient’s goal(s) from their current support plan.

In Plain English:
If an Individual wants to live in their own home, the state will pay someone from our company to come to their place and teach them how to do the things they need to learn how to do.

Supported Employment Coaching

In the Developmental Disabilities Waiver Services Coverage and Limitations Handbook:
Supported employment services provide training and assistance in a variety of activities to support recipients in sustaining paid employment at or above minimum wage unless the recipient is operating a small business. The supported employment provider assist with the acquisition, retention or improvement of skills related to accessing and maintaining such employment or developing and operating a small business.

In Plain English:
There is no "mystical or magical" means of finding a job, but a Job Coach will assist an Individual to fill out applications, and apply for jobs until the Individual gets hired by someone, and then the Coach will teach the Individual the skills they need to keep and maintain the job once they get it.

Supported Living Coaching

In the Developmental Disabilities Waiver Services Coverage and Limitations Handbook:
Supported living coaching services provide training and assistance, in a variety of activities, to support recipients who live in their own homes or apartments. These services may include assistance with locating appropriate housing; the acquisition, retention or improvement of skills related to activities of daily living such as personal hygiene and grooming; household chores; meal preparation; shopping; personal finances and the social and adaptive skills necessary to enable recipients to reside on their own.

In Plain English:
If an Individual is ready to move into their own apartment, a Supported Living Coach assist them to fill out rental applications, obtain funds if necessary to pay for their rent and utility deposits, and learn how to pay their monthly bills and live within their means of income.

Moving Mountains, Inc. is based upon providing Med-Waiver Services to improve the quality of life and outcomes for Individuals with Disabilities. The central theme for our Agency is "Advocacy," defined and developed through the supports provided to the Individuals we serve. Our services help the Individual to become increasingly independent which elevates their confidence level and strengthens the desire for the Individual to achieve their life visions and goals.
